Ontario Ultimate (OU) is the body responsible for handling our insurance. If a player needs to file an insurance claim due to an injury, please be sure to:
- Fill out an Incident Report. If there’s a claim being filed this is obviously an injury worthy of reporting. The insurance company will want to know the details. Make this easier for yourself and the league.
- Download the claim form here
- Fill it out pronto and get a league representative to sign off on it.
- Submit the claim form in the allotted time (30 days). This may necessitate a physician’s recommendation as well, so please have all documentation on hand to ease the process.
